JOB SUMMARY: Responsible for providing high quality patient care, displaying good skill! s in assessment, psychodiagnosis, and treatment planning; conducts group, individual and family therapy; conducts routine discharge planning for Medicare, traditional Medicaid, and non-resource patients; and directs patients to designated community agencies or resources, as necessary. Additionally, he/she is expected to function well as a member of the multidisciplinary treatment team.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
1. Consistently supports the hospitalâs Spirit of Serving Standards, in line with the Mission , Vision and values of St. Joseph Medical Center .
2. Follows the guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI)
3. Promotes a culture of safety for patients and Associates through proper identification, proper reporting, documentation and prevention of medical errors in a non-punitive enviro! nment.
4. Conducts individual psychotherapy, family t! herapy, and group sessions, displaying good theoretical knowledge of psychopathology, family therapy principles and group therapy techniques.
5. Conducts educational group sessions, displaying good theoretical knowledge of subject matter and effective communication skills in presenting material.
6. Completes psychosocial assessments and psychosocial updates in a timely and high quality fashion.
7. Completes all components of treatment plans and documents daily progress on individual problems related to therapy functions.
8. Communicates patientsâ progress to other treatment team members; plans/devises patient goals in conjunction with other treatment team members, including psychiatrist, nursing staff and patients.
9. Works in close conjunction with patient, family, nursing staff, and attending physician.
10. Completes progress notes in a timely and high quality fashion for inpatient and PHP patients.
11. Asse! ss each assigned patient on admission for educational needs, discharge and for after care plans to ensure good transitional management and continuity of care. Review and revise as changes occur and document consistently and timely on the progress notes.
12. Involves patient and family in all treatment/discharge plans. Complete Aftercare Instructions clearly, accurately and timely.
13. Demonstrates knowledge and understanding of all levels of post acute care, and proficiency in facilitation of discharge and transfer to all settings.
14. Consistently has Medicare Important Message and Choice Letter signed.
15. Has knowledge of Mental Health Warrants and Court Process for involuntary admission. Complete court papers for the assigned case load in a timely manner.
16. Enters charges accurately and timely. Turns in charge sheet for reconciliation before end of shift
17. Assists Director with program planning, implementation an! d monitoring clinical activities.
18. Conducts Performance Imp! rovement activities and disseminates results to treatment team. Utilizes results to make recommendations related to program improvements/changes.
19. Monitors patientsâ progress/regression on an on-going basis and recommends treatment changes to primary psychiatrist.
20. Assists in de-escalating patients and displays appropriate CPI techniques.
21. Utilizes age appropriate techniques for the adult and geriatric population.
22. Supportive of the compliance program set forth by IASIS and demonstrated by:
a. Upholds the Code of Ethics and Corporate Compliance.
b. Adheres to and helps to enforce all compliance policies relevant to his/her area.
c. Assures timely compliance education as requested by the Regional Safety & Compliance Officer and/or through corporate initiatives.
23. Sets an example to all staff in their daily activities.
WORK EXPERIENCE:
One year experience working with m! ental health patients is required.
LICENSE/REGISTRATION/CERTIFICATION :
Current licensure as a Licensed Professional Counseling (LPC), or a Licensed Masterâs in Social Work. (LMSW) is required, or Psychologist is required.
Current American Heart Association Basic Life Support certification and CPI certification is required.
EDUCATION & TRAINING:
Masterâs degree in Social Work, counseling, or psychology
SKILLS:
Technical, critical thinking, and interpersonal skills relevant to Behavioral Medicine in order to effectively communicate with physicians, team members, patients and families.
Ability to prioritize work with minimal supervision, in order to independently carry out the duties of the position.
Able to communicate effectively, in English, both verbally and in writing.
Additional languages preferred.
Basic computer knowledge.
