VA TEXAS VALLEY COASTAL BEND HEALTH CARE SYSTEM
The VATXVCBHCS is comprised of the Corpus Christi Outpatient Clinic (CCOPC), Corpus Christi Specialty Clinic (CCOPC-S), Harlingen Outpatient Clinic (HOPC), Health Care Center at Harlingen (HCC), Laredo Outpatient Clinic (LOPC), and McAllen Outpatient Clinic (MCOPC). The VATXVCBHCS is a virtual health care system providing secondary and tertiary health care in medicine, surgery, psychiatry, and rehabilitation medicine. The VATXVCBHCS also operates a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program. Through the utilization of 2 complex inpatient/emergency department contracts, approximately 1600 inpatient admission and 1200 ER visits are managed by VATXVCBHCS. Affiliated with the University of Tex! as Regional Academic Health Campus, the system has an active a! mbulatory care program and serves as a teaching and training site for medical residents. The VATXVCBHCS is responsible for an enrollee population of over 30,000 veterans and 25,000 outpatient visits annually.

DUTIES:

You will serve as a Registered Nurse for the Patient Aligned Care Team (PACT) at the VA Texas Valley Coastal Bend Health Care System (VATXVCBHCS). You will use the nursing process and evidence-based practice to collaborate with the Teamlet and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. You will provide patient and family health education with a focus on self-management,! prevention, and wellness, based on the patientÂ's goals. Demonstrate s! ound clinical knowledge in assessing planning, implementing, documenting, and evaluating care for a designated group of patients across the continuum of care. Triage and apply a collaborative team approach in identifying, analyzing, and resolving patient care problems.
Duties include the following, but are not limited to:
Provide indirect patient care in collaboration with the interdisciplinary team; serve as clinical resource expert; and function as an educator for the team and patient.
Screen patients for social service, home care, and other community care needs; and coordinate or make referrals as appropriate; and seek consultation when indicated.
Appropriately documents own interventions and oversees appropriate health team documentation of patient care.
Keep patientÂ's provider and appropriate case manager aware of patient progress, issues, and/or problems.
Evaluate needs and facilitate the patientÂ's ability to l! earn the principles of self-care; utilize appropriate resources if the patient is unable to grasp the knowledge/skills needed for self-care.
Actively participate in care delivery with priority attention to patient preference.
Promote cost effective use of resources; effective least-expensive strategies expected in non-face-to-face approaches (including telehealth).
Your role will focus on working with an interdisciplinary team to manage a panel of patients assigned to the provider and team. This requires using your clinical knowledge and critical thinking skills to identify, analyze and assist with resolving the patient care issues while using approved protocols and guidelines to facilitate autonomy in providing care.
You will also triage patient calls using the nursing process and applying skills such as motivational interviewing and conducting an accurate assessment to assure the patient receives an appropriate level of care within an a! ppropriate timeframe. You will use your computer skills and document pa! tient interactions within the patientÂ's computerized patient record system (CPRS) and use the TeleCare Record Manager Plus (TRM Plus).
Work Schedule:
8:00 - 4:30 p.m.; Alternate schedules may be necessary to meet the needs of the organization.

EXPERIENCE PREFERRED:
2 - 3 years medical-surgery or primary care experience preferred
1 - 2 years Emergency room or urgent care experience preferred

QUALIFICATIONS REQUIRED:
Citizen of the United States.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency
and
accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia
. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
Must be proficient in spoken and written English

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Maste! r's degree in nursing (MSN) or related field with a BSN and no experience.
Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
PHYSICAL REQUIREMENTS:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients! and objects may be required. The incumbent may be exposed to infected ! patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
