Performs complex (journey-level) social service or case management work involving the care and community! reintegration of youth who have been adjudicated delinquent of felony offenses, committed to the agency by a juvenile court, and placed directly in a low to medium security halfway house or placed in a halfway house as a gradual transition from a TJJD high-security correctional facility back into their home community.

Provides living skills training and coordinates youth participation in activities relating to community reintegration in collaboration with halfway house administrative and direct-care staff, TJJD case managers, and TJJD parole officers. Requires interaction with representatives from community organizations and groups.

Coordinates health care activities to facilitate youth access to medical and psychiatric services and to help ensure the medical and medication needs of youth are met while assigned to the halfway house and the continuity of applicable medical care upon release. These duties require extensive collaboration with the halfway hou! se case managers and with the University of Texas Medical Bran! ch Correctional Managed Care (UTMB/CMC) nursing and health care staff contracted to provide medical, dental, psychiatry, pharmacy, and nursing services for youth in TJJD operated facilities, including onsite care provided in-person and through Digital Medical Services (DMS) sessions.

May be assigned to assist with supervision of daily activities as needed at the halfway house. May train other staff. Works under general supervision with moderate latitude for the use of initiative and independent judgment. May work overtime and may be required to be on call.

REQUIREMENTS:
BachelorÂ's degree with major course work in health care, behavioral science, social science, criminal justice-corrections (not law enforcement or police sciences), education, or related field. One year of full-time wage earning experience that may be any combination of the following: coordinating community resources, case management, or medical services, which may include servic! es for at risk or disadvantaged populations; experience in an adult or juvenile correctional, mental health, or detention setting; or experience in the health care field or involving review of medical records.

OR

High School Diploma or GED. Five years of full-time wage earning experience that may be any combination of the following: coordinating community resources, case management, or medical services, which may include services for at risk or disadvantaged populations; experience in an adult or juvenile correctional, mental health, or detention setting; or experience in the health care field or involving review of medical records. Completed undergraduate course hours may substitute for experience on a basis of 30 course hours for one year of experience.

Preferred:
Education or experience in the health care field or experience with medical records.

Required:
Must have and maintain a valid driver license with an acce! ptable driving record.

Note: Any degree or course hours! required to meet minimum qualifications must be obtained from an accredited educational institution. Experience requirements may be satisfied by the prorated part-time equivalent.

4. Knowledge of mental illness and the emotional components of physical illness, of adolescent development and needs, and of adolescent care practices.

5. Knowledge of rehabilitation methods.

6. Knowledge of individual needs associated with disabilities.

7. Knowledge of medical terminology.

8. Skill in interviewing and in conducting individual needs assessments.

9. Skill in medication administration and use of the pharmacy replacement system.

10. Skill in using logic and reasoning to identify the str! engths and weaknesses of alternative solutions, conclusions, or approaches to problems.

11. Ability to gather, assemble, correlate, analyze, and evaluate information.

12. Ability to convey instructions and explanations and to communicate effectively with youth, co-workers, and supervisors.

13. Ability to assist in planning health care and assessing health care needs.

14. Ability to maintain accurate records and files.

15. Ability to train others.

16. Ability to follow agency policies, rules, and procedures.

17. Ability to prioritize and manage multiple tasks; plan, organize, and coordinate work assignments; and meet deadlines.

18. Ability to work collaboratively with all stakeholders and maintain a professional, courteous demeanor.

19. Ability to process information logically, make decisions, and demonstrate the soundness of those decisions.
