English Language Proficiency required. DUTIES:
Provides direct outreach and pre-release assessment services to re-entering veterans in federal, sta! te and local correctional facilities and will serve as a link between the network, medical centers, and local, state and federal agencies. Has clinical responsibility for providing information, assessment and screening, VA and non-VA linkage and referral, and coordination and follow-up services to veterans during the transition from institution to community. Has program coordination responsibility for developing and maintaining effective relations with local, state and federal correctional personnel for developing and maintaining access mechanisms that identify re-entering incarcerated veterans, and for developing and maintaining access to VA and non-VA programs that are needed for the veteranÂ's community readjustment. Participates in program monitoring and evaluation activity conducted by VAÂ's Northeast Program Evaluation Center.

Fully responsible for the management and coordination of the program which includes developing all aspects of the program and institut! ing any changes in the substance and structure of the program that are ! needed to provide better care to our veterans.

Clinical service effectiveness: overall, provides information, pre-release assessment and re-entry planning services, post-release referral, linkage, and coordination for VA and non-VA services, and community-based follow-up services to re-entering veterans during the transition from prison to community. Establishes respectful, effective relationships with veterans, and with the clinical, custodial, and administrative staff from the range of institutions and agencies, including prisons, involved in re-entry.

Relocation expenses will not be reimbursed.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
a. Citizenship. Be a citizen of the United States.
b. Education. Have a masterÂ's degree in social work [ ] from a school of social work fully accredited by the Council on Social Work Education (CSWE).
c. Licensure. Persons hired or reassigned to social worker positions in ! the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the masterÂ's degree level.

d. Physical Requirements. The evaluation of an individualÂ's ability to perform efficiently and without hazard to self or others will encompass a composite consideration of physical, mental, and emotional ability as related to realistic requirements for the duties and responsibilities to be assumed.
e. English Language Proficiency. Social workers must be proficient in spoken and written English.

Social Worker Program Coordinators and Manager. For Social Worker Program Coordinators, grades are determined by the scope of responsibility and the type, size, and complexity of the assignment.

(a) Experience, Education and Licensure. Individuals assigned as social worker program coordinator or manager must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced! practice skills and judgment, demonstrating progressively more profess! ional competency. They may have certification or other post-masterÂ's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below.

(b) Demonstrated Knowledge, Skills, and Abilities

1. In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.

2. Knowledge and skill in management/administration, which includes consultation, negotiation, and monitoring.

3. Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).

4. Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.

(c) Assignments. The! KSAs shown above apply to all Social Worker Program Coordinator grade levels.

1. GS-12 Social Worker Program Coordinators. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty, such as Spinal Cord Injury (SCI) coordinators, Visual Impairment Services Team (VIST) coordinators, Homeless Veterans Program (HVP) coordinators, Community Nursing Home Program (CNHP) coordinators. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the programÂ's budget.

Prefer: experience working with the homeless and/or incarcerated individuals; VA experience; and, experience working within ! the legal system in Texas.
