Medical Record Technicians (MRTs) organize, process, and maintain medical records and health information. They a! ssemble, review, analyze, code, abstract, maintain, compile, and extract medical record and health data. MRTs check medical records for completeness, accuracy, and compliance with requirements of regulatory and accrediting agencies. Some MRTs maintain clinical registries and work to meet the standards of regulatory and accrediting agencies. Typically MRTs work under the guidance of the Medical Record Administrator.
To be creditable, t he experience must have required the use of knowledge, skills, abilities, and other characteristics (also referred to as Â"core competenciesÂ") associated with current health information or medical record technology.
Medical Coders analyze and abstract medical record data and assign codes using clinical classification systems. Medical coding requires independent judgment based on medical record documentation and personal coding knowledge and skill. Certification is offered by AHIMA as a Certified Coding Associate (CCA), Certified Codin! g Specialist (CCS), Certified Coding Specialist-Physician-base! d (CCS-P), or Registered Health Information Technician (RHIT). Certification is offered by the American Academy of Professional Coders (AAPC) as a Certified Professional Coder Apprentice (CPC-A), Certified Professional Coder-Hospital Apprentice (CPC-H-A), Certified Professional Coder (CPC) or Certified Professional Coder-Hospital (CPC-H) and is highly desired at the higher grade levels.
This position serves as a Medical Record Technician (Medical Coding Auditor) and consults with the coding staff and medical staff on issues regarding coding of outpatient and inpatient care. This position serves as an MRT (Medical Coding Auditor) and supports the Chief of Health Information Management Service (HIMS), Assistant Chief of HIMS and all sections in HIMS with coding related revenue activities. This position is also responsible for providing professional consultation, administrative assistance and health record support to the HIMS Manager at the North Texas VA Healthcare System! in addition to the MRT duties covered by the standard.
Serves as a liaison between the coding and billing offices to resolve problems/issues that occur and coordinates with all necessary offices for resolution. Purpose is to ensure an effective program based on approved coding and billing guidelines. Identifies problematic areas and provides feedback/guidance to Coding Supervisor or directly to coders as appropriate.
Supports clinical and administrative staff by providing coding and billing education in addition to providing reference materials and guidelines that support appropriate documentation and the perspective payment system. Coordinates with Consolidated Patient Accounts Center (CPAC) staff by providing feedback on pertinent issues. Serves as a resource person for multiple departments by interpreting federal regulations, assisting with code assignments and ensuring standardization in departmental procedures. Plans and conducts educational sessions for codin! g, ancillary and clinical staff. Keeps current with federal guidelines ! and publications in order to establish and promote coding and payor reimbursement strategies.
Works with CPAC to educate staff concerning denials that require coder follow-up. Conducts ongoing data validation audits. Analyzes findings from all reviews and recommends additional coding instruction as appropriate. Provides periodic feedback to HIM staff regarding coding trends that are identified through this process.
Serves as the primary point of contact for CPAC staff in resolution of coding, billing and accounts receivable problems. Participates in work groups, committees and task forces to provide a coding perspective on reimbursement and revenue issues.

Complies with all coding guidelines and regulations. Guidelines consist of various regulations, guidance, and laws pertaining to specific program areas. These include, but are not limited to rules and regulations promulgated by the Office of Management and Budget, Department Veterans Affairs, HIPAA, Cente! r for Medicaid and Medicare Services (CMS), Coding Clinic, Current Procedural Terminology ( CPT) Assistant, and numerous others. The incumbent must perform assigned tasks in accordance with the professional criteria for gathering, analyzing, and evaluating information, preparing fact-finding studies concerning management processes and making recommendations to top-level management. The work must be accomplished in concert with the appropriate stipulations of all departmental and government statutes and regulations. In some cases, it is the role of this position to determine the intent and verify the interpretation of such guidance in order to recommend implementation at the Healthcare System. The various guidelines will not always provide the appropriate guidance and the incumbent must ensure the Healthcare SystemÂ's programs meet the general intent of those guidelines by adapting them to the Healthcare SystemÂ's specific situation and unique requirements.

Qualifications & Evaluations

ENGLISH LANGUAGE PROFICIENCY : In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient! care positions unless they are proficient in basic written and spoken English. BASIC REQUIREMENTS:
Citizenship. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified candidates in accordance with chapter 3, section A, paragraph 3g, this part.)

Experience and Education
(1) Experience. Two years of experience that demonstrates the applicantÂ's ability to perform the work, or provides familiarity with the work. Six months of the required 2 years of experience must have provided the knowledge, skills, abilities, and other characteristics (KSAs) needed to perform MRT work.
OR,
(2) Education. Successful completion of an associateÂ's degree with a major field of study in medical record technology/health information technology which was accredited by the American Health Informatio! n Management Association (AHIMA) at the time the program was completed.!
OR,
(3) Experience/Education Combination. Equivalent combinations of experience and education are qualifying. The following educational/training substitutions are appropriate in this provision for combining education and experience:
(a) Successful completion of an associateÂ's degree in a field of study other than medical records/health information will substitute for 18 months of the required experience.

(b) Successful completion of a course for medical technicians, hospital corpsmen, medical service specialists, or hospital training, obtained in a training program given by the Armed Forces or the U.S. Maritime Service under close medical and professional supervision, may be substituted on a month-for-month basis for up to 1 year of experience.
(c) Successful completion of a post-high school course of study in medical record technology, which includes courses in anatomy, physiology, and medical record techniques and procedures, may substitute on a! month-for-month basis for up to 1 year of experience.
( d) Successful completion of post-high school courses for medical administrative personnel, obtained in a closely supervised training program given by the Armed Forces, may be substituted for experience on a month-for-month basis for up to 6 months provided the training program included courses in anatomy, physiology, and medical record techniques and procedures.

GRADE DETERMINATIONS:
GS-09
(a) Experience. One year of experience comparable to the next lower grade level of GS-08 and must fully meet the Knowledge Skills and Abilities (KSAs) at that level (below).
1. Ability to analyze the medical record to identify all pertinent diagnoses and procedures for coding, and to evaluate the adequacy of the documentation. This includes the ability to read and understand the content of the medical record, the terminology, the significance of the comments, and the disease process/pathophysiology of! the patient;

2. Skill in reviewing medical record documentati! on and assigning current versions of the classification systems required in the current position, such as ICD and CPT; and

3. Advanced knowledge of the full scope of coding and abstracting including inpatient discharges, surgical cases, diagnostic studies and procedures, outpatient encounters, and inpatient professional fees for a highly diversified range of specialties and subspecialties, such as orthopedics, neurosurgery, cardiology, gastroenterology, plastic surgery, spinal cord injury, blind rehabilitation, anesthesia, acute and long term psychiatry including addiction treatment, hospice, ambulatory surgery, and other types of care.
Medical Record Technician (Medical Coder) Positions. This grade is considered to be above the full performance level. Certification at this grade level is highly desired. In addition, the candidate must demonstrate the following KSAs:
a. Demonstrated Knowledge, Skills, and Abilities
(1) Ability to collaborate and coordin! ate tasks with individuals outside the work area and at higher organizational levels who require persuasion and negotiation;
(2) Ability to work independently to accomplish a variety of responsibilities including setting priorities, organizing and adjusting workflows, utilizing problem-solving
techniques in the accomplishment of work;

  • (3) Ability to apply expert technical knowledge and skills in medical records/health information, HIPAA, or a related area in a complex work environment;
  • (4) Comprehensive knowledge of the most complex and difficult medical record technician areas; and
  • (5) Knowledge of qualitative and/or quantitative methods for assessment and improvement of medical record functions.
  • Note - Must demonstrate the potential to acquire the assignment specific KSAs.
