Foster Family Case Manager

Location: San Antonio, Texas

Description: BCFS is looking of Foster Family Case Manager right now, this position will be presented in Texas. Detailed specification about this position opportunity please give attention to these descriptions. Major Responsibilities:
Coordinate and process the application and discharge procedure for new foster children with the Program Director or Designee

Function as the liaison b! etween BCFS and children’s schools as necessary, tending as well to compensatory programs.

Coordinate and efficiently document all Plan of Service meetings within the guidelines of all monitoring agencies along with all other areas pertaining to normal casework.

Ensure that children’s recreational, spiritual, and therapeutic needs are being met and that their rights are being protected and documented.

Meet regularly with foster parents to discuss children’s progress and problem-solve.

Meet regularly with foster children, individually and as groups to assess their needs, discuss their progress and problem-solve.

Coordinate, respond, and give supervision to services involving family members and reunification efforts, caseworkers, and/or probation officers, therapists, medical providers, attorneys, child advocates, sponsors, mentors, PAL workers, judges, and psychiatrists.

Assist children with transitional issues such! as college preparatory requirements, after-care living skills! , after-care employment, mental health services, etc.

Report on the safety status or other discernible needs of foster residents.

Keep an agency funded cellular phone and respond to crisis situations as needed.

Reconcile DFPS payment schedules with BCFS records in tandem with the Program Director or designee.

Document and provide monthly progress summaries for DFPS, Juvenile Probation, and other monitoring agencies as needed.

Facilitate the adoption process as necessary for families. This includes legal paperwork, training, and home study updates.

Develop community and governmental resources for each foster home and/or child.

Develop and coordinate Family Sponsorships and/or mentors for as many children as possible, particularly if residing in a group home.

Maintain an adequate number of personal training hours and attend all required meetings.

Plan, document, post, and instruct children, staff! , and foster families as to emergency procedures for each foster home.

Respond to all agency related calls and correspondence in a timely fashion.

Maintain an amicable, responsive, and appropriate relationship with all staff, children, and clients of BCFS.

Exercise diligence in keeping the Program Director, supervisory chain of command and/or the President/CEO informed regarding any issues which might potentially compromise the integrity of the program.

Perform all other duties as assigned by Program Director, supervisory chain of command, or the President/CEO.

Other job duties as assigned.

Qualifications

Education: A minimum of a bachelor’s degree in a human relations field, such as Social work or psychology, from an accredited college or university

Experience: One to Three years case management experience preferred

Requirements:
A valid Texas driver license and a dr! iving record that demonstrates safety and responsibility, as well as a ! clear criminal history check, and three positive professional references.

A desire and ability to work flexible hours and be responsive to staff, children, and families in need of support at all times via phone.

A working knowledge of computer data entry and retrieval.

The ability to communicate and problem-solve effectively with children, families and staff, and network to the agency’s benefit within the community and with a variety of outside organizations.

The desire and ability to adhere to the ethics code of BCFS, and receive and implement the directives and critique of a supervisor.
