CRISIS PSYCHIATRIC TRIAGE RN

Location: Denton, Texas

Under direction of the Program Manager of Psychiatric Triage and within federal, state and local guidelines provides quality nursing care to individuals using effective triage and assessment skills while maintaining ethi cal standards of nursing and client confidentiality at all times. Delivers care to individuals utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with individuals and other health care team members while maintaining standards of professional nursing. This position is grant funded.
Promotes the physical and mental health of individuals by applying the nursing processes. This is done as a member of the interdisciplinary treatment team and has the goal of assisting individuals to assume responsibility of their own health. The nurse coordinates and monitors medical services, including psychiatric and medication services, in accord with the requirements of regulatory agencies and in response to the individual’s needs. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Essential Duties

Works collaboratively with MCOT team memb ers and Crisis Residential team members to ensure all necessary Level of Care 0 and Level of Care 5 services are provided including but not limited to: crisis follow-up, case management, pharmacological management and med training/support and nursing assessments.

Communicates effectively following procedure with Denton County MHMR daytime, weekend or evening teams as appropriate to provide continuity of care.

Ensures follow-up on 100% of clients screened within 24 hours.

Will consult with psychiatrist or APN as needed regarding symptoms and side effects for clients. May conduct physical, developmental, and psychosocial assessments to sometimes determine eligibility for health services.

Refers per protocol to the most effective and least restrictive approach to the behavioral health emergency which may include consulting with the on-call

Ensures all risk of harm related paperwork is submitted to assessment tray immediately followin g ROH assessment.

Ensures all clinical documentation is completed within Center guidelines.

Documentation to be completed as services occur but no later than 2 business days.

Contact supervisor or designated back-up for clarifications of procedures or clinical assistance as needed.

May provide direct nursing services and/or telephonic services in the individual’s living environment, psychiatric clinic or alternate care setting, for individuals being served in the psychiatric triage facility as part of a Psychiatric Triage Team.

Facilitate with individual and with community agency to carry out least restrictive environment.

Participates through a collaborative effort in developing and implementing a comprehensive crisis plan that provides the most effective and least restrictive available treatment. This plan is based on the provisional psychiatric diagnosis and will incorporate all requirements listed in the protocol an d or procedure related to crisis services including reassessment needs adjustment to individual's response, coordination, resolution of crisis and transference to appropriate package.

Completes CANS/ANSA to determine whether or not client qualifies for crisis package services. (LOC 0 and LOC 9).

Follows appropriate medication procedures

Provides telephone follow ups to individuals as needed and as directed by Denton County MHMR daytime MCOT teams. Instructs, counsels, and assists clients in meeting their health care needs. Provides symptom management and education with the focus of resolving crisis while assisting the individual to be an active participant in their treatment.

Practices work place safety procedures.

Attends regularly scheduled meetings.

Contact supervisor or designated back-up for clarifications of procedures or clinical assistance as needed.

Provides emergency intervention when necessary accordin g to Department of Human Services (DHS) and Center rules and procedures.

Maintains appropriate gas level in center vehicle.

Completes vehicle check out and sign in log.

Completes log binder in vehicle.

Ensures completion of tracker on each shift.

Ensures caring letters are sent out appropriately.

Perform job duties in a safe manner and report any unsafe practices or conditions and demonstrates awareness of potentially dangerous situations.

Keep daily service logs on all direct care activities.

Education and or Experience

A high school diploma or equivalent and graduation from an accredited Nursing School required. Must have and maintain a current RN license with the Board of Nurse Examiners in the State of Texas. Three (3) years previous nursing experience and three (3) years prior experience working with persons with major mental illness preferred. Experience working on a multi interdisciplinar y team preferred.

Registration Certification Licesure other Qualifications

Must have and maintain a current R.N. license in good standing with the state of Texas Board of Nurse Examiners.

Must have valid Texas driver's license, proof of auto liability insurance or waiver of insurance, and maintain an acceptable driving record as defined by the Center’s insurance carrier requirements.

All job specific training must be completed within 30 days of hire.

Must be able to perform the techniques of Prevention and Management of Aggressive Behavior (PMAB).

Must be able to perform Cardiopulmonary Resuscitation (CPR).

Ability to be mobile within the community to provide direct nursing services if needed.
