The Shared Psychiatric Services Program offers access to mental health services, comprehensive clinical case managers, as well as resources to reduce barriers to care, such as transportation ! assistance and access to medications.
Shared Psychiatric Services is a collaboration of LifeWorks, Communities in Schools and SafePlace. Funded by the St. David's Foundation, the program offers psychiatric services to children who are enrolled in services of the collaborating agencies. Goals include:
- Providing client-focused, strengths-based psychiatric services to clients
- Reducing barriers to accessing psychiatric services for clients
- Empowering clients to take an active role in their care
DUTIES
- Coordinates implementation of psychiatric services for internal and external partner clients. Duties may include intake, case management, follow-up and resource referral.
- Transla! te communication between the clients and SPS staff, including ! psychiatrists for non-English speaking.
- Communicates with collaborating community agencies regarding services and appointments.
- Explains Patient Assistance Program (PAP) procedures to clients and assists clients in accurately completing program applications and obtaining required documentation; submits PAP application to providers for review and signature and forwards completed applications and related paperwork to pharmaceutical companies.
- Serves as liaison between Patient Assistance Programs, SPS staff, eligibility and partnered pharmacies.
- Collaborates with community agencies to advocate for client population needs.
- Enters client information in the ETO database, runs necessary reports, maintains client information and confidential client files, and follows-up with clients as required; provides information regarding data gathered and entered into ETO to the SPS Program Evaluator.
- Maintains case records and documentatio! n as appropriate, including service plans, progress notes, intake reports, and incident reports.
REQUIRED:EDUCATION AND EXPERIENCE:
- Requires a Bachelor's degree in behavioral science or a related field;
- Requires 2+ years of related experience; or
- Equivalent combination of education and experience.
PREFERRED:
Experience providing client support in a medical setting(3 years); Experience coordinating services/collaborating with other community agencies (3 yrs); Experience with administrative tasks and data entry (3 yrs); Experience with medical translation between English and Spanish *Coordinates implementation of psychiatric services for internal and external partner clients.
