The Sr. Social Worker assists the healthcare team in meeting the psychosocial needs of patients and their families, enabling them to utilize medical care in order to achieve their optimal level of care. The Sr. Social Worker also assists with discharge planning for the patient in order to maintain the delivery and continuity of care.

Masters of Social Work required
Licensed Clinical Social Worker required
Experience
5+ years applicable experience required
Inpatient social work experience strongly preferred
Individual and/or group therapy / counseling experience strongly preferred
Experience working with the geriatric and/or psychiatric patient populations strongly preferred
