Salary Grade: 9 Position Summary Overview:
To assist patients and their families in making psychosocial adaptations specific to their diagnosis, while facilitating the optimal utilization of health care services and community resources in accordance to the age and developmental level of the patient. Routine decisions include the assessment of patient and family's psychosocial needs, resolving patient and family related problems and referrals to TCH-IDS and community resources and services. Conducts a psychosocial assessment, within one working day of receiving the inpatient consult or three working days of receiving the outpatient consult, as evidenced by documentation in the medical record and/or team observation/feedback. Communicates with the patient, family and team as evidenced by documentation in the medical record and/or team observa! tion/feedback. Develops an action plan with the patient, famil! y and team as evidenced by documentation in the medical record and/or team observation/feedback. Educates patient and family regarding the diagnosis and community resources as evidenced by documentation in the medical record and/or team observation/feedback. Engages patient/family in problem solving as evidenced by documentation in the medical record and/or team observation/feedback. Makes and/or assists family to make resource referrals as evidenced by documentation in the medical record and/or team observation/feedback. Provides appropriate therapeutic support (individual/family/group/grief counseling, crisis intervention), either by direct service or by referral, as evidenced by documentation in the medical record and/or team observation/feedback. Obtains information, consultation and professional support from inter-disciplinary team members as evidenced by documentation in medical record and team feedback. Contacts Children¿s Protective Services, as appropriate, as evi! denced by documentation in the medical record and/or team observation/feedback. Coordinates inter-disciplinary child protection team meetings and provides psychosocial information pertinent to formulating a team care plan as evidenced by team meeting records.
Knowledge & Skills
Must have knowledge of medical social work, psychosocial models and theories and community resources. Must have the ability to detect abused children and coordinate with community resources to protect children believed to be abused. Ability to conceptualize the psychosocial needs presented by the assigned patient population, to intervene with patient/family behavior, to coordinate community resources and to handle crisis and complex psychosocial situations. Ability to handle grief. Ability to demonstrate initiative, good judgment and problem solving skills. Ability to demonstrate excellent professional listening and communication skills.
