The Intake Coordinator is responsible for coordinating the delivery of care throughout the continuum of care for assigne! d patient population. Functions include conducting pre-admission clinical interviews to evaluate psychiatric, physical, psychosocial and financial needs and resources of patients/families. The Intake Coordinator is a member of the multidisciplinary team and works closely with physicians, clinical therapists, financial counselors, and ancillary staff to actively facilitate those functions associated with arranging of admissions of patients to inpatient, partial, day treatment or intensive outpatient programs. This position also researches, identifies and implements initiatives and opportunities to improve multidisciplinary communication.
Essential Duties and Responsibilities
Resource Management

  • Application of severity of illness/intensity of service criteria for patients seeking psychiatric admission or continued stay in all levels of care
  • Interacts with patients and physicians to explore the most appropriate clinical setting to meet the patient! and family needs
  • Interfaces with appropriate departm! ents to maintain appropriate authorizations for services
Transition Planning
  • Collaborate with team members to develop a transition plan appropriate to the patient's needs and ability to pay
  • Serve as consultant to patient and family and collaborates with staff to determine after care service recommendations/resources
  • Convene and conduct interdisciplinary conferences to solicit input from clinical and business team members on selected patient
  • Serve as liaison to community resources
Clinical Management
  • Attend weekly rounds and participate as needed to become a visible and contributing team member
  • Facilitate referrals to the appropriate areas to expedite care, treatment and services
  • Provide feedback to referral sources in order to enhance continuity of care
  • Establish lines of communication and develops a network of the usual services and disciplines required by the typical pati! ent
  • Collaborate with physician, patient and family, and nurse to identify probable post-acute needs
  • Participates in the development, implementation, evaluation, and ongoing revision of clinical pathways and initiatives to improve quality, continuity, and cost-effectiveness
Communication/Process Improvement
  • Communicate patient needs to appropriate professionals (i.e. clinical therapists, physicians, clinical dietitian, clinical manager)
  • Follow-up on communication
  • Communicate continually with patients and families, physicians, care team members, and third-party payers to facilitate coordination of clinical activities and to enhance the effect of a seamless transition from one level of care to another across the continuum
  • Works collaboratively with other departments and services to define and study areas of inefficiency and participates in process improvement projects
Qualifications
Education Master's prepared mental health clinician - social work, counseling,! psychology, family therapy or BSN Nursing with experience in psychiatric triage and case management
Licenses & certifications
Professional license as LMFT, LCSW, LPC, or RN to practice in the state of Texas
Experience
Minimum 3-5 years of job related experience
Specific knowledge, skills and abilities
Physical and emotional health adequate for performance, which may require extended or flexible work hours, rapid assessment and response to patients, families and employees
Analytical ability to effectively prioritize, assess, plan, coordinate, and evaluate the care of patients and their families to achieve quality and cost effective outcomes
Effective interpersonal skills with positive relationship building; effective written/verbal communication; ability to negotiate with interdisciplinary team members, physicians, families, payor sources and peers
Knowledge of evolving healthcare systems, including but not limited to, funding sources, th! ird party reimbursement, contractual arrangements, managed care and continuum of care issues
Evidence of self-direction, flexibility, adaptability, creativity, ability to prioritize efforts, and effective time management
Knowledge and experience with personal computers and software
Is knowledgeable and acts in accordance with laws and procedures regarding patient confidentiality
Physical demands
Sedentary - Exerting up to 10lbs. occasionally or negligible weights frequently; sitting most of the time.
