Mesa Springs Hospital is seeking dynamic individuals for our Care Coordination positions as well as Care Coordination Manager.
Mesa Springs will be a new 72 bed free-standing ! psychiatric and addiction hospital providing inpatient and outpatient services. The outpatient services will include PHP, IOP and aftercare programs. This newly constructed 50,000 square foot building is located on 9.4 acres in Fort Worth TX, and is scheduled to open in winter 2013. It will be a beautiful building designed with a warm and attractive interior. Our mission is to provide the highest quality treatment services to adults and their families suffering from mental illness and addiction. Description
Care Coordinators
assess the needs of walk-in and phone callers to ensure their referral to the service or re! source to best address those needs. Specialist completes initi! al psychiatric assessment on patients seeking treatment of substance abuse, dual diagnosis or psychiatric or emotional disorders and presents symptoms to psychiatrist for best recommendation of care. Specialist interprets treatment to patient and family to help reduce fear and other attitudes obstructing acceptance of care and continuation of treatment. Specialist refers and triages patients to other sources of treatment when appropriate. Specialist answers all inquiry calls regarding treatment and the facility, presents treatment options, documents calls and sets appointments for callers seeking treatment. Specialist works with the business office to ensure smooth transition into the admission process. Mobile assessments may be required.
Manager ensures quality therapeutic care to patients seeking treatment of substance abuse, dual diagnosis or psychiatric or emotional disorders. Manager oversees the assessment of the needs of walk-in and phone callers and the com! pletion and completes initial psychiatric assessment on patients seeking treatment. Manager supervises the answering and answers all inquiry calls regarding treatment and the facility, presents treatment options, documents calls and sets appointments for callers seeking treatment. Manager creates, revises, and implements the schedule of the assessment and referral office. Manager assesses the needs of walk-in and phone callers to ensure their referral to the service or resource to best address those needs. Manager completes initial psychiatric assessment on patients seeking treatment of substance abuse, dual diagnosis or psychiatric or emotional disorders and presents symptoms to psychiatrist for best recommendation of care. Education: Masters Degree from an accredited college or university, in Social Work or Rehabilitation Counseling. RN licensure may be used in place of masters degree.
Experience: Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients preferred. Must have strong clinical assessment skills.
License: Current unencumbered clinical social work or counseling license.
Additional Requirements:
CPR certification and Crisis Prevention Training (CPI) within 30 days of employment and prior to any patient contact. May be required to work flexible hours and overtime.
